Chronic Care or Disease Management is a system of coordinated healthcare interventions and communications for populations with conditions in which patient self-care is significant. Managed Care Concepts offers a comprehensive chronic care suite of services through its WELL-Managed Chronic Care product after members have been identified by our own OMNI tool.

Why do Chronic Care Management?

If employers focus solely on high risk populations, they are missing the 20% of low risk who are moving to medium or high risk and they miss the 40% of medium risk who are migrating to high risk. Goal: to keep low-risk individuals at low risk and keep them from migrating to high risk. An employer cannot win where there is continual flow to high risk.
